Internship Forms

Regarding Internship and Professional Practices;

• The internship evaluation form has been updated. This form will be filled out by the company at the end of the internship and must be submitted in a sealed envelope.
• The vocational practice report has been updated. This form will be filled in by our students during vocational training at the enterprise.
• Internship report has been updated. This form will be filled in by our students during the internship.
• External stakeholder opinion form has been newly added. This form will be filled in by the enterprises during vocational training in the enterprise.
• Vocational training course interim report form has been added. This form will be filled in by students during vocational training at the enterprise.
• These forms, which must be submitted, must be submitted by e-mail during the pandemic period, and after the pandemic period, by e-mail and by hand to the academic staff in the internship commission.
• Our students should not forget to submit the declaration and commitment form and internship acceptance form before the internship.
